Health and Care Portsmouth Blueprint

Health and Care Portsmouth is the ambitious programme which is changing the way we plan and deliver health and social care in the city.

The aim is to help people stay healthy and independent, and to give people a network of health and care services which are focused on meeting the needs of patients, not the organisations providing the care. It brings together our CCG, Solent NHS Trust, Portsmouth City Council, the Portsmouth Primary Care Alliance, and other partners.

Everyone talks about the need for 'joined up' services, Health and Care Portsmouth is delivering them.

Here are just a few examples of the work underway:

  • "Physio First" - helping people suffering back and joint pain to get quick access to specialist help, without a GP appointment
  • Enhanced care home support team - improved support for care home residents, and better support for care home staff
  • Integrated out-of-hours primary care - Portsmouth-based clinical staff providing evening and weekend care, able to access patient notes to improve safety and quality
